Corrective Actions and Recommendations
Item PY10 – Drawing Revision
A typographical error in the vessel capacity indicated on the drawing was identified. The drawing was revised and re-uploaded to the KEA server.
Item TK04 – Design Calculation Revision
The design pressure in the Design Data section of the strength calculation was corrected from 0.5 MPa to 0.4 MPa, and the revised calculation was re-uploaded to the KEA server.
Pressure Test Procedure – Holding Time
The required holding time was missing from the Pressure Test Procedure. The procedure was revised to include the applicable holding time.
Autoclave – Process Check List
The Jacket Coil fabrication process was missing from the Process Check List for the autoclave items. The checklist was revised accordingly.

5. Hydrostatic Test – Clamp/Bolt/Nut Assemblies
Temporary clamps and nuts were used during the hydrostatic test to prevent damage to the original components. However, their material and strength could not be verified, and some nuts were assembled with multiple washers due to improper fit.
As these components were confirmed to be pressure-retaining parts, the manufacturer was advised to use the original components specified in the design.
The original clamp/bolt/nut assemblies were installed, and the hydrostatic test was repeated with a 10-minute holding time.
Shell-to-Cone Joint – Welding Quality
Welding quality concerns were identified at the shell-to-cone joint. For future projects, additional NDE is recommended to verify the integrity of this welded joint.
Alternatively, consideration should be given to changing the cone connection to a straight-flange-type configuration to improve fabrication and welding quality. This recommendation was also discussed and confirmed with the design engineer.
